This 4-part series offers attendees the opportunity to understand and develop better documentation and billing procedures to improve patient care and enhance opportunities for provider reimbursement. 

Part 1: The Audit

This section will look at the Chiropractic Documentation Checklist, which may be used in Medicare audits of chiropractors.  Each section of the checklist will be broken down and discussed, as well as looking at the key parts of the claim form chiropractors use for Medicare beneficiaries.

Learning Objectives:

  1. Discuss the Chiropractic Documentation Checklist used in Medicare audits.
  2. Illustrate the CMS claim form and the critical components for Medicare billing.
  3. Demonstrate the proper use of the Advance Beneficiary Notice of Non-Coverage form (ABN).
  4. Illustrate the PART system of documentation required by Medicare.

Part 2: Assessing Pain

This section will discuss the CMS requirements for assessing pain and the required components of a thorough patient history.

Learning Objectives:

  1. Discuss the Problem Oriented Medical Record.
  2. Illustrate proper history taking and its role in patient management.
  3. Demonstrate various methods of assessing pain intensity for a patient.
  4. Illustrate two different methods of assessing pain frequency for a patient.

Part 3: Assessing Function 

This section will discuss the CMS requirements for assessing function and the concept of outcomes-based patient management.

Learning Objectives:

  1. Discuss the CMS requirements for functional outcome assessments.
  2. Illustrate various functional outcome questionnaires that can be used with patients.
  3. Demonstrate how to score and track functional outcome questionnaire scores.
  4. Discuss the frequency of functional outcome questionnaires required by CMS.

Part 4: The Daily Visits 

This section will discuss the CMS requirements for assessing function and the concept of outcomes-based patient management.

 Learning Objectives:

  1. Discuss the concept of documenting separate multiple incidents/episodes.
  2. Illustrate the necessary components to be documented for each new incident/episode.
  3. Discuss the patterns of diagnosing and the two types of diagnoses required for each Medicare claim.
  4. Define the Assessment part of the SOAP note and the required components.
